On Nov. 6, new forms for DOT physicals will be required.

The new forms are designed to help guide doctors who are not experts on the medical requirements for commercial drivers through the process and help keep drivers with serious medical problems from slipping through the cracks in the system. Among other changes, the rules for what can disqualify someone from driving a truck are actually printed right on the form.
The form was published in the Federal Register on October 5, 2000. 65 FR 59363. The Federal Motor Carrier Safety Administration does not have the forms. They may be obtained from printing firms, state trucking associations and other sources. Here are a few sources:
