NTSB: Trucker, Carrier, FMCSA at Fault in Fatal Truck Crash

The National Transportation Safety Board determined the probable cause of a fatal truck crash two year ago was a trucker's fatigue – but it said a carrier's failure to adhere to HOS rules and “inadequate safety oversight by the Federal Motor Carrier Safety Administration" contributed as well.

Judge Denies Retrial to Trucking Company in Amtrak Crash

A Nevada trucking company has been refused a retrial stemming from a fatal 2011 accident in which an Amtrak train was struck by a truck along U.S. 95, causing the deaths of 6 people, including the trucker.

NTSB Report Finds Value in On-Board Video Systems

An NTSB report has concluded that on-board video systems can “provide valuable information for evaluating the circumstances leading to a crash,” if the cameras are positioned properly and if the systems are adequate for recording in low light.

The National Transportation Safety Board has issued its preliminary report about the April 10 crash between a FedEx Freight truck and a bus in California killing 10.

NTSB Says Train and Tractor-Trailer/Parade Float Crash and Deaths Due to Poor Planning

A fatal accident in which a freight train struck a tractor-trailer hauling a parade float in Midland, Tex. last November was caused by the failure of both the city and the parade organizer to address the risks associated with routing a parade through an active grade crossing and it serves as a cautionary tale to any trucking operation that volunteers equipment or people for such events.

Truck Driver Was Distracted Before Bridge Crash, NTSB Says

A truck driver reported that he was distracted just before he ran into the car in front of him, pushing it off of the Chesapeake Bay Bridge into the water 27 feet below.
